Tournament Release & Liability Waiver
MEDICAL RELEASE
The undersigned gives instructors, staff, and tournament officials full power to authorize medical or other emergency treatment of the registered competitor, subject to medical advice. If under 18, the parent or legal guardian acknowledges they have legal right to grant this authority.
ASSUMPTION OF RISK
Martial arts tournaments involve physical contact and active competition. Abrasions, bumps, sprains, strains, fractured bones, concussions, or other sports-related injuries are risks of competition. By signing, the competitor or guardian fully assumes all responsibility and risk.
RELEASE OF LIABILITY
The undersigned holds harmless tournament organizers, judges, referees, host schools, facility owners, and Martial Arts America LLC from any claims arising out of participation.
